ABSTRACT

@#Metformin is currently the first-line drug for the treatment of diabetes. In addition to its hypoglycemic effect, it has also been found to have other potential effects, such as anti-inflammatory, odontogenic differentiation-promoting, osteogenic differentiation-promoting, and antitumor effects. Previous studies have shown that metformin can promote the healing of periapical lesions, and its mechanism may be related to the promotion of osteogenic differentiation and the induction of dental pulp cell differentiation by activation of adenylate-activated protein kinase by dimethyldiphosphate. Clinical indexes, such as the probing depth, attachment loss level and probing bleeding index, were significantly improved in patients with periodontitis treated with metformin, which may play a role in the prevention and treatment of periodontal disease by promoting the proliferation, migration and osteogenic differentiation of periodontal ligament stem cells. Metformin has been proven to inhibit the growth and proliferation of tumor cells and plays an important role in the prevention and treatment of oral tumors such as oral squamous cell carcinoma. At present, research remains in the in vitro and animal experimental stage, and the related mechanism needs to be further explored. Clinical trials remain in the evaluation of clinical indicators, so large-scale, long-term, multicenter, randomized controlled clinical trials need to be further developed

ABSTRACT

@#Sonodynamic therapy (SDT) is a cutting-edge method for the biological effects of ultrasound combined with sound-sensitive agents. In recent years, SDT has been a concern of experts and scholars in the oral field, and a series of experimental studies has been carried out. We will introduce the progress of SDT in the field of stomatology from three aspects: the therapeutic mechanism of SDT, the application of SDT in the oral field, and the current situation and future of SDT in the treatment of oral diseases. It is currently believed that singlet oxygen theory, cavitation effects, and induction of apoptosis are the main therapeutic mechanisms. The research of SDT on oral disease prevention mainly focuses on oral tumors (especially squamous cell carcinoma and osteosarcoma) and infectious diseases (such as periodontitis, maxillofacial infection, and oral mucosal disease). Although the current research is still in the experimental stage, with the continuous exploration of sonosensitizers and nanotechnology, SDT will provide great help for the clinical prevention and treatment of oral diseases in the future.

@#Treponema denticola is an important pathogenic Treponema pathogen in the human oral cavity. Early studies have found that Treponema denticola is closely related to the occurrence and development of periodontal diseases. With the development of technical methods in recent years, many studies have shown that Treponema denticola not only can participate in periodontal diseases through a variety of mechanisms but also can play an important role in the development of various oral diseases. Treponema denticola is detected in high concentrations in peri-apical diseases and peri-implant diseases, and its surface protein is also prevalent in oral tumor samples. This paper reviews the research progress of Treponema denticola in periodontal diseases, pulp peri-apical diseases, peri-implant diseases and oral tumors, and summarizes the relevant mechanisms. For example, Treponema denticola can cause immune regulation disorder, destroy the epithelial barrier, induce bone absorption, promote the occurrence and development of inflammation through a variety of surface proteins, including chymotrypsin-like protease complex (CTLP), major outer sheath protein (Mosp), Td92, and LOS. It can also escape complement-mediated killing effects through surface FhbB lipoproteins and promote the occurrence and development of oral tumors by regulating the tumor microenvironment. These theories provide a theoretical basis for further understanding the development of oral diseases, controlling the infection of Treponema denticola, and exploring more effective treatment strategies.

ABSTRACT

After the resection of oral tumor, defected maxillofacial structure caused functional difficulties including phonetics, mastication and esthetic aspects. In this cases, implant retained prosthesis can contribute to the functional enhancement. Regardless of the success rate in grafted bone, however, the soft tissue usually had a shape which was susceptible to inflammation. Moreover, infected graft bone presented rapid destruction. For success of the prosthetic treatment, adequate soft tissue treatment and frequent recall check are the essential factors to the successful implant prognosis.

ABSTRACT

Rânula congênita uma rara malformação cística visualizada na cavidade oral. É um pseudocisto habitualmente localizado no espaço sublingual entre o músculo milo-hioideo e a mucosa da língua. Relata-se um caso de gestante de 24 anos, G3P2, com idade gestacional de 29 semanas, encaminhada por conta de polidrâmnio e grande massa de cavidade oral de natureza cística.


A congenital ranula is a rare cystic malformation seen in the oral cavity. This pseudocyst is normally located in the sublingual space between the mylohyoid muscle and the lingual mucosa. A 24-year-old woman, gravida 3, para 2, at 29 weeks' gestation was referred to our institution because of polihydramnios and large oral mass.

ABSTRACT

The aim of this study was to assess the peri-implant parameters and evaluate the clinical status with the survival of dental implants in body of maxilla after treatments of oral tumor. A follow-up examination included 27 patients who underwent the ablative tumor and (or) reconstructive surgery during a 5-year period. The follow-up protocol included clinical examination, radiological evaluation, and an interview using a standardized questionnaire. The reasons related to implant failure were studied by comparing the amount of failure with the value of marginal bone resorption, probing pocket depth, and plaque index using statistical t-test. The relationship between smoking and implant failure was analyzed statistically by chi-square test. The results showed among the 112 implants observed after implant loading, 29 have failed with the failure rate being 22.14 %. There was no significant correlation between the peri-implant status and the implant failure (P>0.05), however, the association of smoking and implant failure was statistically tested (P<0.05). It was suggested that the association of peri-implant status and implant failure in the maxilla after tumor surgery can't be statistically tested, however smoking was still a mainly significant factor.

Objective:To study the genetic toxic effect of smoking on human oral mucosa,and the DNA damage of exfoliated oral mucosa cells.Methods:The DNA damage of exfoliated oral mucosa cells was investigated by single cell gel electrophoresis(SCGE)in 12 cases of malignant tumor,19 cases of benign tumor and 10 health controls.There were 24 smokers and 17 non-smokers among them.The tail length and frequency of comet cells were used to measure DNA damage.SPSS and SAS software were used for statistical analysis.Results:Malignant tumors had a longer tail length and higher frequency of comet cells than benign tumors and health controls,and the difference was statistically significant(P0.05).The DNA damage of exfoliated oral mucosa cells in smokers was more serious than that of non-smokers(P

ABSTRACT

Objective:To study the relationship between smoking and P53 protein expression in oral tumor tissue.Methods:The expression of P53 protein in oral tumor tissue was detected in 22 smoking cases and 32 nonsmoking cases by immunohistochemical SP method. SAS software was used for statistic analysis.Results:The positive expression rate of P53 protein in smokers (90.91%) was higher than that in nonsmokers (46.88%,P0.05). Conclusion:Smoking may stimulate the mutation of p53 gene and play an important role in the carcinogenesis of oral tissues.

ABSTRACT

Nowadays, there are a lot of evidence that mutation of the p53 tumor suppressor gene is one of the most common genetic abnormalities in neoplastic progression. In this study, we analyzed 20 specimens of oral tumors (squamous cell carcinoma 14 cases, ameloblastoma 3 cases, adenoid cystic carcinoma 2 cases, malignant schwannoma 1 case) using polymerase chain reaction and direct sequencing which used an automated DNA sequencer and software for detection of mutations. Polymerase chain reactions were performed with 4 sets of primers encompassing exon 5, 6, 7, 8, and direct sequencing method was employed. The results were as followings. 1. We detected 10 piont mutations out of 20 specimens (50%). 2. The genetic alterations included 7 mis-sense mutations resulting in single amino acid subtitutions, 2 silent mutations, 1 non-sense mutations encoding a stop codon. 3. Mutations were mostly in exon 7(7 out of 10 mutations, 70%) and involved codons 225, 234, 235, 236, 238, 247. 4. Therse were 4 cases of T-->A transversion, 2 cases of C-->A transversion, A-->G transition, 1 case of C-->G, T-->G transversion respectively. 5. We could find out point mutations more conveniently using PCR-Automated Direct Sequencing method.
